Gavin Shan 27e27782f7 sPAPR/IOMMU: Fix TCE entry permission ...

The permission of TCE entry should exclude physical base address.
Otherwise, unmapping TCE entry can be interpreted to mapping TCE
entry wrongly for VFIO devices.
